Please note that this form is intended for OPTAVIA Coaches who wish to transfer one of their frontline Clients to a different Business Coach. 

• If a Client wishes to change their OPTAVIA Coach, please click here to begin the process. 
  • NOTE: You must first log into OPTAVIA.com with the credentials that matches the Client making the request.
• If you are a Coach wishing to transfer multiple of your frontline Clients, please use the Bulk Client Transfer Request Form.
• If you are a Coach requesting to transfer to a different Business Coach, within your first 30 days of enrollment, please use the Sponsor Transfers Within 30 Days of Enrollment form.
